If you child requires perscribed or over the counter medication at school, you will need to fill in the appropriate paperwork. If the medication is a prescribed medication a doctor/pharmacist/practicing nurse will need to fill in the medication form B. If the medication is an over the counter medication then parents can fill out the Medication form A. Form A and B can be found on the SZapp under forms and documents or you can collect from the office. All medication will need to be in the original packaging and within the expiry date.
